From La Porte Hospital head Northeast on W Lincolnway toward Madison Street. Turn left onto US-35 N/ Pine Lake Avenue. Turn left onto Johnson Rd. Turn left onto US-20. Turn right onto Cleveland Ave, we are located on the right at 4300 Cleveland Avenue past the Fire Department.
